At-home saliva test · CLIA-certified lab

The link between gum inflammation and blood sugar runs both ways.

It is one of the most firmly established connections in the research: poorly controlled blood sugar can worsen gum inflammation, and the inflammation from periodontal bacteria can make blood sugar harder to control. The Oral Balance Test measures the bacterial side of it — sixteen organisms, from one saliva sample you give at home.

Why the mouth is part of this picture

Inflammation is the part of this you can actually measure.

Most people managing blood sugar have never been told their mouth is involved, and no routine panel looks at it. Measuring which oral bacteria are present — and how strong the protective ones are — turns one half of a documented relationship from an assumption into a number.

A two-way relationship, not a one-way claim

Research describes influence in both directions — glycemic control affecting gum inflammation, and periodontal inflammation affecting glycemic control. Our report labels this a well-established association, meaning strong and repeatedly confirmed. It is still an association, not a statement about any individual.

The bacterial side is measurable

You already have numbers for the metabolic side. The oral side usually goes unmeasured — nobody checks which periodontal organisms are present, or at what level. That is the part this test puts a number on.

Something to raise with both clinicians

Your report names what was found and comes with questions written to be handed over. Useful to a dentist, and worth mentioning to whoever manages your blood sugar — this connection sits between two specialties that rarely compare notes.

What you get back

The report devotes a page to exactly this question.

You can read all five pages before you order — walked through section by section, including the systemic-associations page and the limits it states about itself.

  • A page on systemic associations Heart, blood sugar, brain, joints and colorectal — each labelled by how strong the evidence is, with the ones relevant to your sample first and the rest shown in grey as reassurance.
  • All sixteen organisms, named Each with a category, a detection level, and a one-line note — including the periodontal organisms this association is actually about.
  • Numbered citations to the literature Every association is cited. The blood-sugar one rests on Preshaw et al., "Periodontitis and diabetes: a two-way relationship," Diabetologia 2012.
  • Questions to ask, and when to retest A short list to hand to your dentist, and a retest window so you can tell whether anything you change actually moves the numbers.

What this is, and what it isn't

Straight answers

Does this test my blood sugar?

No. It measures bacterial DNA in saliva — sixteen specific organisms and how much of each. It does not measure glucose, A1c, insulin, or anything metabolic. It is not a diabetes test and it is not a screening tool for one. What it measures is the oral half of a relationship the research describes; the metabolic half is your clinician's territory and stays there.

Should I change how I manage my blood sugar based on this?

No — and please don't. Nothing in this report is a reason to alter medication, diet, or monitoring, and we would be well outside our lane to suggest otherwise. It is information to bring to the people who manage your care, not instructions that bypass them.

Will treating my gums improve my blood sugar?

We won't promise you that, and you should be wary of anyone who does. The research describes an association that runs in both directions — it does not let us tell you what will happen in your particular case. What this test can do is show whether periodontal organisms are elevated in your sample, which is a specific, checkable thing rather than a general worry. Whether addressing it changes anything metabolic for you is a question for your clinicians, and a retest will at least tell you whether the bacterial side moved.

How strong is the evidence, really?

Our report labels this one well-established, which in its own glossary means the association is strong and repeatedly confirmed in research — as distinct from supported (consistent and growing) or emerging (plausible, still under study). The primary citation is Preshaw et al., "Periodontitis and diabetes: a two-way relationship," Diabetologia 2012. The report also states plainly that none of these labels describe your personal likelihood of developing anything — only whether the organisms studied in connection with it were found in you.

Is this a diagnosis?

No. The Balance Test measures your oral microbiome. It doesn't diagnose gum disease, diabetes, or any other condition, and it isn't a substitute for a dental exam or for medical care. Results are for adjunctive use and are meant to be read alongside a clinical examination and your history.

How long until I get results?

Your report is ready within 2 business days of your sample reaching our lab — about a week from the day you mail it, once transit both ways is counted. It's the same validated test dental practices order, run in our own CLIA-certified, COLA-accredited lab. It has not been cleared or approved by the FDA.

Which states do you ship to?

We ship the at-home Balance Test to most US states, but not yet to AK, CA, CT, HI, MD, MA, NJ, NY, RI. Those states apply extra requirements to at-home lab testing, and we've held them back from this first launch rather than cut corners. We're working through them.

If you're in one of those states and want the test, ask your dental practice — they can order OraPath panels for you directly.
